Open Access Policy Waiver Form

Rarely, a publisher requires its authors to opt out of the University of Kansas Faculty Senate’s Open Access Policy for a given article.  The policy provides a means for this to be done. To opt out and obtain a waiver for a specific article, simply fill out this brief form notifying the Provost’s Designate of your need and provide the citation for the paper for which you need the waiver. You will be sent immediately an email confirmation of the waiver granted.

As permitted by the policy adopted by the KU Faculty Senate, in which I have granted KU a license with respect to my scholarly articles, by clicking "Submit," below, I hereby request a waiver of the policy for the article listed below.

Please provide as many publication details for the article as available at this time. Include title, journal, volume/issue, and anticipated publication date.
