Open Access Policy License Waiver Form

Occasionally, a faculty author may need to opt out of the license granted to University of Kansas via the Faculty Senate’s Open Access Policy for a given article. The policy provides a means to do so. To obtain a waiver, submit this brief form notifying the Provost’s Designate, providing the citation for the paper for which you need the waiver. You will immediately receive an email confirmation of the waiver granted.

Per the KU Faculty Senate Open Access Policy, in which I have granted KU a license with respect to my scholarly articles, by clicking "Submit," below, I hereby direct that the waiver be granted for the article provided below.

Please provide as many publication details for the article as available at this time. Include title, journal, volume/issue, and anticipated publication date.
